Position Summary:

The Caregiver provides care and supervision to residents in accordance with California

Department of Social Services (DSS) Title 22 regulations, the Community’s policies and

procedures, and each resident’s individual care plan. The Caregiver supports residents with

activities of daily living (ADLs), observes and reports changes in condition, maintains a safe and

clean environment, and promotes resident dignity, independence, and quality of life.

Reports to: Resident Care Coordinator

Essential Duties and Responsibilities

Resident Care & Supervision:

● Provide resident care based on assigned caregiver level and individual resident care

plans.

● Provide direct care with activities of daily living (ADLs) including bathing, toileting,

dressing, grooming, mobility, and transfers as applicable.

● Maintain resident dignity, comfort, privacy, and respect during care delivery.

● Support residents through companionship, cueing, observation, redirection, and light task

assistance.

● Escort residents and provide supervision during movement within the community and

participation in activities.

● Assist with resident activity programs and community engagement as assigned.

Meals & Nutrition Support:

● Escort residents to and from the dining room while encouraging independence when

appropriate.

● Assist with meal service and dining room duties as directed, including monitoring and

reporting intake concerns.

Observation, Reporting & Documentation:

● Observe residents for changes in physical, mental, or emotional condition.

● Monitor and promptly report changes in resident condition, incidents, or safety concerns

to supervisors.

● Document care activities, observations, incidents, and changes in condition accurately

and timely in accordance with Community policies and procedures.

Infection Control, Housekeeping & Safety:

● Follow infection control and safety standards in accordance with Community policy and

Title 22 requirements.

● Demonstrate proper handling of equipment, soiled linens, bedpans, urinals, and perform

adequate hand hygiene.

● Maintain cleanliness, organization, and safety of resident rooms and common areas.

● Provide laundry and housekeeping services to residents as assigned.

● Avoid loss, breakage, and waste of supplies and equipment.

● Ensure doors and alarm systems are properly secured at all times.

Shift Responsibilities & Communication:

● Receive report from the previous shift and medication technician.

● Complete physical rounds at shift change, including resident status review, trash, laundry,

and assigned hall/section.

● Review assigned duties, including shower schedules and care assignments.

● Report all health, safety, and sanitation concerns to the appropriate supervisor.

Emergency Response & Administrative Support:

● Respond to emergencies and follow Community protocols.

● Be familiar with and follow fire safety, disaster preparedness, missing person, and

emergency procedures as required by Title 22.

● Provide front desk coverage as assigned, including monitoring call and alarm systems

and providing clerical support as requested.

● Maintain respectful and professional interactions with residents, families, visitors, and

staff.

● Demonstrate reliability, punctuality, and attention to detail.

● Maintain resident confidentiality and support resident rights at all times.

● Work independently and cooperatively as part of the care team.

● Wear name tag at all times.

● Adhere to and convey the Community’s philosophy of maintaining elements of self-care

and encouraging independence.

Other Duties:

● Perform other duties as assigned by management consistent with the caregiver role.

Language Requirement:

This position requires the ability to effectively communicate in English, both verbally and in

writing, in order to perform essential duties such as understanding care plans, documenting

resident services, following safety protocols, and interacting with residents, families, and team

members. Reasonable accommodation will be considered as required by law.

Qualifications & Requirements:

● Minimum age: 18 years (or as required by state regulations)

● TB clearance (per state requirement)

● First Aid and CPR Certification (per state requirement)

● Criminal background clearance and fingerprinting (LIC 508)

● Health Screening Report (LIC 503)

● Previous experience preferred but not required

● Willingness to work with elderly population

Universal Precautions:

Exposure to blood and other bodily fluids is likely. All caregivers must follow infection control

procedures, including proper use of personal protective equipment (PPE), hand hygiene, and safe

handling/disposal of contaminated materials, in accordance with Title 22 and Community policy.

Physical Requirements:

● Ability to lift residents minimum 35lbs.

● Ability to stand, walk, bend, lift, and perform repetitive tasks throughout the shift.

● Ability to assist residents with mobility, positioning, and transfers in accordance with

resident care plans and Community safety policies.

● Ability to lift, carry, push, or pull up to 35 pounds independently; heavier lifting requires

assistance, team lifts, or mechanical equipment.

● Ability to use proper body mechanics and transfer equipment such as gait belts,

wheelchairs, and mechanical lifts as required.

● Ability to respond quickly and effectively to emergencies, including assisting with

evacuation procedures.

● Ability to perform housekeeping tasks, including making beds, handling laundry, and

maintaining clean and safe resident environments.
